pjw 99be2d6e79 internal/lsp: bring lsp protocol stubs up to date ... The only substantial change is data types for CallHeirarchy. util.ts has changed the hash, and adapted to a new source layout, plus the usual pointless whitespace changes. code.ts has learned a little more about typescript ASTs.
