Heschi Kreinick 7042ee646e internal/lsp/source: always look up mapper when building ranges ... Any file could have //line directives in it, which means that we should never trust a mapper that was looked up for a whole file. Remove the range conversion helpers that accepted a mapper and look it up on the spot.
