Alan Donovan b929f3bf4d internal/span: make NewRange accept File, not FileSet
span.NewRange now accepts a *token.File and two token.Pos.
It is the caller's responsibility to look up the File
in the FileSet, if necessary (it usually isn't), and to
ensure the Pos values are valid.  Ditto NewMappedRange.

This reduces the creep of Snapshot into functions that
have no need to know about it. Also the bug.Report call
in NewRange has been pushed up into the caller and
logically eliminated in all but one case.

I think we should aim for the invariant that functions that
operate on a single file should accept a *token.File, not
a FileSet; only functions that operate on sets of files
(e.g. type checking, analysis) should use a FileSet.
This is not always possible: some public functions
accept a FileSet only to re-lookup a single file already
known to the caller; if necessary we could provide token.File
variants of these.

This may ultimately allow us to create a new FileSet per
call to the parser, so that Files and FileSets are in
1:1 correspondance and there is no global FileSet.
(It currently grows without bound, on the order of kilobytes
per keystroke.) FileSets containing multiple files,
needed when we interact with the type checker and analysis,
may be temporarily synthesized on demand from a set of Files,
analogous to mmap'ing a few files into a blank address space.

Also:
- replace File.Position(pos).Line by File.Line(pos)
- replace pos == token.NoPos by pos.IsValid()
- avoid fishy token.Pos conversions in link.go
- other minor simplifications

Robert Findley 37590b385d gopls: remove usage of golang.org/x/xerrors
As of golang/go#50827, gopls no longer supports building at 1.12, and so
usage of golang.org/x/xerrors can be replaced with the native support for
error wrapping introduced in Go 1.13.

Remove this usage as a step toward eliminating the xerrors dependency
from x/tools.

Marwan Sulaiman 70c3ea29e2 gopls,internal/lsp: Implement method stubbing via CodeAction
This CL adds a quickfix CodeAction that detects "missing method"
compiler errors and suggests adding method stubs to the concrete
type that would implement the interface. There are many ways that
a user might indicate a concrete type is meant to be used as an interface.
This PR detects two types of those errors: variable declaration and function returns.
For variable declarations, things like the following should be detected:
1. var _ SomeInterface = SomeType{}
2. var _ = SomeInterface(SomeType{})
3. var _ SomeInterface = (*SomeType)(nil)
For function returns, the following example is the primary detection:
func newIface() SomeInterface {
	return &SomeType{}
}
More detections can be added in the future of course.

Marwan Sulaiman 1a7ca93429 internal/lsp/source: update SuggestedFixFunc to accept source.Snapshot
As part of the work for implementing method-stub code generation,
this CL updates the function signature of source/command.go's SuggestedFixFunc
so that a command can operate on the entire source.Snapshot to analyze
and change multiple packages and their files.

Pontus Leitzler bb69444e35 internal/lsp: send "extract variable" edits ordered
Edits from the code action "extract variable" wasn't ordered.

The reason this wasn't picked up by the tests (and in fact, the test
failed when edits were in order) is that multiple edits to the same file
was sent as multiple "DocumentChanges" with a single edit each.

By updating ApplyFix() to consolidate edits to the same file into a
single DocumentChange with multiple edits, we avoid the undefined(?)
behaviour of how clients should handle multiple DocumentChanges to the
same file & version.

Suzy Mueller 46d1522a5d internal/lsp: add extract to method code action
"Extract method" allows users to take a code fragment and move it
to a separate method. This is available if the enclosing function
is a method.

Rebecca Stambler 09a00c1ab1 internal/lsp: fix support for SourceFixAll code actions
Some of the refactoring changed the way that we label code action kinds,
and we need to add quickfix and fixall kinds for each diagnostic type.
Support a per-kind suggested fix, and fix a small issue in setting the
analyzer for a fixall code action.

Rob Findley 8aef11fa67 internal/lsp: switch to the new command API
Fully switch to the new generated command API, and remove the old
dynamic command configuration.

This involved several steps:
 + Switch the command dispatch in internal/lsp/command.go to go through
   the command package. This means that all commands must now use the new
   signature.
 + Update commandHandler to use the new command signatures.
 + Fix some errors discovered in the command interface now that we're
   actually using it.
 + Regenerate bindings.
 + Update all code lens and suggested fixes to new the new command
   constructors.
 + Generate values in the command package to hold command names and the
   full set of commands, so that they may be referenced by name.
 + Update any references to command names to use the command package.
 + Delete command metadata from the source package. Rename command.go to
   fix.go.
 + Update lsp tests to execute commands directly rather than use an
   internal API. This involved a bit of hackery to collect the edits.
 + Update document generation to use command metadata. Documenting the
   arguments is left to a later CL.
 + Various small fixes related to the above.

This change is intended to be invisible to users. We have changed the
command signatures, but have not (previously) committed to backwards
compatibility for commands. Notably, the gopls.test and gopls.gc_details
signatures are preserved, as these are the two cases where we are aware
of LSP clients calling them directly, not from a code lens or
diagnostic.
